Here is a list of the of items which you will need when you apply for a mortgage. Feel free to print this form and use it as a checklist to assist you in preparing for your application.

1. Original copies of last two years W-2's and Income Tax Returns
*2. All paystubs for the most recent month showing borrower's name, social security number, and year-to-date earnings
3. Employer's name and address for past two years
4. Three months most recent bank statements for all accounts
5. Account numbers, payment amounts and balances on installment loans
6. Social Security card and drivers license- we will make copy
7. Mailing address(s) for past seven years
*8. Divorce decree and proof of court ordered child support
*9. Settlement statement on recent real estate sale
10. If you own rental property, we will need your current lease(s). If you don't have a lease, we will need tax returns.
*11. Copy of notes receivable
*12. Bankruptcy papers
*- if applicable
Additional Information: (If Refinancing)
1. Copy of deed
2. Prior Title Policy
3. Copy of Homeowners Insurance
4. Survey- if less than 10 years old and it must be certified
If Self Employed:
1. Last two years tax returns- with all schedules, business and personal
2. Year-to-date profit and loss
If Construction:
1. Copy of the deed- if you own the lot
2. Plans and specifications- fully executed builder package
If VA Loan:
1. Copy of DD214 (member 4 showing honorable discharge)
2. Original certificate of eligibility- if you have it
